CASE MANAGEMENT is the entry point into the service delivery system for people with disabilities and special needs. Case Managers work with individuals and/or families to meet these needs. FCDSNB coordinates with other agencies such as DSS, Mental Health, Vocational Rehabilitation, and local schools to provide services for individuals meeting eligibility requirements. The Case Manager ensures that services needed are accessed by the individual. Family support enables individuals with disabilities and special needs to remain in their homes and community. Supports are offered to those eligible.

EARLY INTERVENTION is a family focused in-home service for children up to age five. An early interventionist helps families understand their child's development. Case Management is provided in accordance with an Individualized Family Service Plan. FCDSNB's Early Intervention Program serves children and families eligible for BABYNET. S.C.'s statewide Early Intervention Program.

CENTER-BASED SERVICES provides a safe and healthy environment for people to develop social and personal care skills for more independent and productive lives. Also, Participants may receive competitive wages doing contract work.

EMPLOYMENT SERVICES provides assistance in sustaining employment. This service provides wages and opportunities to interact with other workers. Employment services include job coach, enclaves, mobile work crew, and day activity opportunities.

RESIDENTIAL SERVICES offers individuals the opportunity to live in the community in a home-like environment under the supervision of trained staff- in community training homes. In addition, personalized care and individualized mining are provided for three or four individualism this setting. Ask a service coordinator for more information.

CONSUMER & FAMILY INVOLVEMENT is the desire of the Board to provide the highest quality services to individuals with disabilities and special needs in Fairfield County. To determine the quality and need of services, the Board depends on the input and feedback from consumers, families, and other community agencies. Consumer Review Groups and Parent Organizations are a means by which the Board receives specific information to plan needed enhancement of and development of services.

INDIVIDUALIZED SUMMER SERVICES provides financial assistance for families whose children attend activities with non-disabled children. It pays for registration or activity fees, transportation, additional staff, summer daycare, admission fees for recreational activities, individual summer activities such as swimming lessons, horseback riding, etc. Other family support includes physical and occupational therapies, personal care and homemaker services, private duty and home health nursing, and environmental modifications when eligible.

HUMAN RIGHTS AND SELF ADVOCACY primary mission of the FCDSNB is to preserve and promote the basic dignity and human rights of individuals with disabilities and special needs. The individuals are entitled to the same rights available to all citizens in the community and in the least restrictive environment.

STAFF DEVELOPMENT General Orientation and enhanced training is offered to ensure quality. Staff Development provides and maintains certification for employees in courses required to prepare them to meet state regulations.
